Submit the packet
Download the prequalification packet, complete it as fully as possible, and send it in with the required attachments. Extra pages with explanatory detail are welcome if the form runs out of room. Our administrative team reviews every submission and will likely reach out with questions.

Include these with your packet
- Signed NDAThe non-disclosure agreement we send by email after your submission is accepted.
- W-9Current, signed IRS Form W-9 for your company.
- Licenses & certificationsA list of all trade licenses and certifications your firm holds.
- OSHA 300 / 300AYour most recent OSHA injury and illness recordkeeping forms.
- Certificate of insurance (COI)A current certificate of insurance for your company.
- Business insurance declarations pageAuto coverage, general liability, workers’ comp, umbrella, and any other active policies.
- CSI division codes listThe CSI codes covering the trade scopes your firm performs.
